Commonheart Palliative Care

Palliative care is specialized medical care that focuses on improving quality of life for individuals with serious or chronic illnesses. Palliative care addresses the whole person—managing physical symptoms like pain, nausea, and fatigue, while also supporting emotional, social, and spiritual needs. Unlike hospice care, it can be provided at any stage of illness and alongside curative treatments.

Palliative Vs. Hospice

While both palliative and hospice care aim to enhance comfort, palliative care supports patients who are still pursuing treatment. Hospice care is intended for those with terminal conditions and focuses on comfort rather than cure.

The Team

Our palliative care team is led by a nurse practitioner who works with your primary care provider to manage symptoms and coordinate care. A medical social worker is available if help is needed to identify supportive services.

Who Can Benefit?

This care can benefit people living with cancer, heart or lung disease, liver or kidney failure, Parkinson’s disease, stroke, or other serious conditions. It can help manage symptoms such as pain, anxiety, depression, constipation, shortness of breath, fatigue, and sleep issues.

Who Pays For Palliative Care?
