Who We Are
Commonheart Home Health and Hospice operates as a service of Illuminus, a not-for-profit organization and a faith-based community open for all, sponsored by the Western District of the Moravian Church.
With a tradition of caring since 1969, we welcome patients in need of hospice services and their families regardless of age, faith, and financial resources.

Serious Illness Support
Managing pain/symptoms of chronic illness. The goal is to improve quality of life during treatment.
