Commonheart Hospice

We’re humbled and grateful to share in the meaningful moments that come with a family’s end-of-life journey. We know you have more concerns than you can count. We’re here to help you through this. Our hospice team provides compassionate healthcare, emotional and spiritual support, bereavement counseling and end-of-life services focused on the comfort and well-being of terminally ill individuals and their families.

Hospice Services

Commonheart Hospice utilizes a team approach to each person’s care, with the patient and family being the primary focus in developing an individualized careplan that best meets their needs and wishes. We begin by providing a free, no obligation evaluation. From there, the Hospice team—consisting of the patient’s physician, Hospice physician, Registered Nurses, Social Workers, Chaplains, Certified Nurse Assistants and Volunteers—works closely to assure that everything we do focuses on promoting comfort and meeting the physical, emotional, and spiritual needs of the patient and family.

Each member of our Hospice team is thoroughly screened and trained to deliver professional, compassionate care. We are committed to helping each person enjoy each day to its fullest potential, making memories that will be cherished for a life time.

Who is eligible for hospice?

Anyone with a terminal illness and an anticipated life expectancy of six months or less and a care plan focused on comfort rather than a cure.

Who pays for hospice?

The Medicare and Medicaid Hospice benefits cover expenses related to life-limiting conditions. Most insurance plans cover some or all of the expenses related to hospice. Even if you don’t have insurance, we can help. Commonheart never turns anyone away for an inability to pay.

It is difficult to find words to express the work of our mother’s Hospice team. Our family has now put him to rest and we are reflecting on how thankful we were to keep him at home with Commonheart Hospice for over four months. Last Wishes Program

Because of generous donations, Commonheart is able to coordinate personalized events for our families to fulfill their last wishes. Our team works closely with families to discover what gives them joy. We then arrange the transportation, tickets and whatever accommodations are needed, so that patients and families are able to spend quality time together, creating memories and sharing love.


Through our Last Wishes Program we are able to help create special memories for our hospice patients and their families. We recently granted wishes for two people to watch a golf tournament while two others chose to attend a Major League Baseball game. In addition to coordinating the events, the foundation paid for specialized medical transportation. Commonheart's team arranged for the individuals and their families to attend a Milwaukee Brewers games and attend The American Family Steve Stricker Golf Tournament held at the University Ridge Golf Course in Madison, Wisconsin. Without this program providing the specialized transportation and event coordination, these memories would not have been possible.

Volunteer Opportunities

Although our hospice team is made up of extraordinary nurses and caregivers, counselors and social workers, we know we can’t do it alone. We need the help of volunteers like you. Helping someone through their end-of-life journey is one of the most courageous and impactful gifts you can give.
