Who We Are.

Commonheart Home Health and Hospice is a service of Illuminus–a not-for-profit organization and a faith-based community open for all, sponsored by the Western District of the Moravian Church, which has a tradition of caring since 1969. Patients in need of hospice services and their families are welcome, including people of all ages, faiths, and financial resources. Every day we honor our commitment to care for the whole person, not just their physical health, but their emotional, spirtitual and social well-being. At Commonheart the work we are called to do is anything but common. It is our honor and joy to provide holistic care to all.
